Usually within three months of your due date, we advise that you pre-register at the hospital. If you pre-register, check-in will be easy as you will not be inconvenienced with forms to fill out while you are in the midst of your baby’s pending debut!

You can print out our admissions form and fax it to the hospital phone-fax number or pre-register now online and check it off your long list of “Things To Do Before the Baby Arrives”!

If neither one of these options suits you, we will give you the form in the office to fill-out and happily fax it for you.
